Defensive Charting: A Legal Perspective on the Do's and Don'ts of Documentation - recorded and presented on December 14, 2023


Many health professionals struggle with creating adequate and thorough charting. By the time a case reaches an attorney's desk, there are often omissions, misspellings, inconsistencies, or failures in following up. Examples will be used to help illustrate charting's impact in the judicial setting and how to avoid common pitfalls associated with poor documentation.

Doug Bitner, JD is an attorney in Indianapolis and a partner in the law firm Stoll Keenon Ogden. His practice focuses on defense of health care professionals, with a specialty of representing medical professionals who are sued by incarcerated or detained individuals for medical malpractice or violation of their civil rights. He is a highly rated speaker at NCCHC conferences and has given many presentations on risk management and litigation for jails and prisons.
