Coordination of Care for Complex Medical Conditions

This webinar presents the highest level of challenge for coordination of care: considerations for patients who require multiple specialists with conditions like cancer. Topics include scheduling and coordinating multiple off-site appointments; on-site follow-up after off-site specialty consultations; and using technologies like telemed or eConsult. Learn how to maximize efficiency on calls with on-site and off-site clinicians. With the sickest patients, failure to communicate and follow-up can have serious consequences; make sure you have the correct tools and resources in place.

Educational Objectives
