Coordination of Care: Emergency Department Visits and Hospitalizations 


Building on the learnings of webinar 1, this webinar will take the discussion further into specific considerations when patients must go to the emergency department or hospital. The session will discuss communication issues regarding transferring clinical data during emergencies, emergency visits that result in hospitalization, and planned hospitalizations. Highlights include addressing unplanned workups and treatments during hospitalizations, utilization management, patient hand-off, and coordinating and scheduling follow-up care. It’s not a matter of if your patients will need ED or hospital care, when it happens, you need the tools and strategies to be effective.

• Evaluate strategies for ensuring comprehensive information transfer for patients requiring an ED visit

Describe best practices for coordinating care with hospitalist/surgical teams

Review how utilization management can assist with coordination of care for hospitalized patients


Presenter:

Dr. Eloy Ituarte, MD is an endocrinologist with extensive experience and training in population health, holding an MD from the USC Keck School of Medicine and a Master's in Population Health Management from Johns Hopkins. His experience in correctional medicine includes working as the Medical Director of the Washoe County Detention Center in Nevada for over five years. In addition to correctional settings, he has practiced in ambulatory care centers, acute care hospitals, and the emergency setting. He currently serves as the Chief Medical Officer at the Reno-Sparks Tribal Health Center.
